In your store's Admin area, go Go to the Design > Blocks→ Blocks
Forms block allow you display in Storefront any custom form made with Design > → Forms Manager. Add additional contact forms, online surveys and more in just a few minutes without writing code.
1. create new Form
go to Design > → Forms Manager page.
2. create new Form block
fill the following fields:
Custom Block Name : block name visible in Admin > Design > Admin → Design → Layouts
Custom Block Title: name shown on Storefront
Use block wrapper (appearance): recommended value is Automatic
Use block's frame: set ON to display frame
Custom Block description: internal use only - not required
Form: Select available form source.
3. Add new form block to Layout
added to desired page in Admin > Design > Layouts. Admin → Design → Layouts
